Gosnells Family Practice is seeking a Diabetes Educator to join our team on a part-time contractor basis.

This is a great opportunity to work within a busy, supportive general practice that is transitioning to bulk billing, with strong patient demand for chronic disease management.

About you

·       Credentialled Diabetes Educator (CDE) (or working towards)

·       Current AHPRA registration (if applicable – RN or allied health)

·       Strong communication and patient engagement skills

·       Ability to work independently as a contractor

·       Experience in general practice (preferred but not essential)

Key Responsibilities

·       Provide diabetes education and management support

·       Develop care plans for patients with Type 1, Type 2 & gestational diabetes

·       Support insulin initiation and glucose monitoring

·       Work collaboratively with GPs and allied health professionals
