Clinical Nurse Consultant – Diabetes Educator (CNC2)

An exciting opportunity is available for a dynamic and motivated Diabetes Nurse Educator to join the outpatient Chronic & Complex Care Team at Orange Health Service.

About the Role
As a Clinical Nurse Consultant (CNC2), you will work as part of a highly skilled multidisciplinary team delivering patient-centred, evidence-based care to people with complex diabetes needs.
Your role will include:
Providing advanced clinical nursing services to clients with diabetes
Delivering self-management education through face-to-face clinics (individual and group), supported by telehealth, and some inpatient services
Supporting continuity of care across hospital, community and primary health settings
Participating in service planning, evaluation and quality improvement activities
Developing and reviewing local policies, procedures and education resources
Guiding practice development and contributing to education and research
Acting as a clinical leader, mentor and resource for staff and students
You will receive a comprehensive orientation and ongoing support from experienced managers within a well-established, collaborative team.
About the Team
The Chronic & Complex Care Team sits within the broader Primary & Community Health service and is known for its strong multidisciplinary approach. The team works closely with hospital services, community health and external partners to deliver integrated models of care that promote wellbeing and reduce preventable hospital admissions.
