Join our team as a Nurse Clinical Specialist, delivering expert care and education to improve chronic condition management across remote NT communities.

 

The Role

As the Nurse Clinical Specialist (Preventable Chronic Conditions), you will provide specialist clinical outreach care to remote clients and specialist support, advice, and education to PPHC staff with the specific focus on improving outcomes in early detection and management of preventable chronic conditions in a culturally sensitive manner.

 

You’ll be accountable for providing sound professional advice and for assessment, planning, review, evaluation, and coordination of comprehensive client care and/or case management.

  About You

To be successful in this role, you’ll have:

  • Current registration with Nursing and Midwifery Board of Australia or eligible for same.

  • High-level clinical knowledge and skills in Preventable Chronic Conditions or related field, supported by strong decision-making and problem-solving skills.

  • Commitment to a learning culture through clinical guidance and mentoring.

  • Ability to travel to remote communities by 4WD or light aircraft as required.

  • Hold a current NT driver’s licence (or eligible to obtain) and be confident driving a manual vehicle in regional and remote settings.
