IPC Health is one of the largest providers of community health services in Victoria. IPC Health is a not-for-profit company providing a diverse range of medical, allied health, psychosocial and aged care services to the community. We are able to provide diabetes education services under Community Health funding as well as GP Management Plans (GPMP) and Team Care Arrangements (TCA). Fees and Cots: Appointments provided under Community Health funding are subject to a co-payment, the payment structure can be found on our website Diabetes Education (ipchealth.com.au) Appointments under GPMP/TCA referrals are bulk billed with no co-payment. Altona Meadows Campus: Diabetes Exercise Group – Every Monday 10am (30 mins exercise, 30 mins education), $7.90 per session, call clinic for more information on number provided
