All Care Diabetes Service is specialising in diabetes management and aiming to enhance the lifestyle of people living with diabetes. Tarak is a highly skilled Credentialled Diabetes Educator with extensive experience in managing all ages people living with Type 1, Type 2, and Gestational Diabetes. Tarak is competent with diabetes technology including insulin pumps, GCMS, and ongoing assessment and management. Tarak has proven experience in managing diabetes for NDIS client and provide thorough education to client, carer and family members. Tarak has good partnership with NDIS providers. Tarak can offer home visit with extra charge up to 10 KM of distance. Within the role, Tarak has developed a strong partnership with General Practitioners, Endocrinologists, and the Allied Health team to provide holistic and patient-centered care. Tarak is highly motivated and self-driven in empowering patients to self-manage their health condition with the use of the contemporary evidence-based practice. |
